Is Grandstream aware of the problem?

They certainly are!

There have been several postings in the Community Forums in past years.

IPv6/Ipv4 February 2016

Users post:

I consider this a bug - you can’t register to 2 separate systems on the different IP stacks.

I tried to set account 1 up to an IPv4 box and account 2 up
to an IPv6 box, the only one that would work depended
on whether I had prefer IPv4 or IPv6 - if I selected prefer
v6, then the v6 worked, if I selected prefer v4,
then the v4 account worked

Grandstream's response:

That behavior is from our current network implementation. 
Only IPv4 or IPv6 will be forced to used by the phone
at a time depends on your internet protocol setting

Grandstream is obviously using their definition of the word 'prefer', which differs from the one used by the rest of the English-speaking world.
